CONTEST RUNS FEB. 1ST, 2012 THROUGH MARCH 31ST, 2012
I. SPONSOR(S)
Priority Health Group(hereinafter referred to as PHG or Sponsor).
II. ELIGIBILITY
Sweepstakes are open to all legal Oregon residents 18 years and older with legal identification.
Any individuals (including but not limited to employees, consultants, independent contractors, and interns or employee’s immediate family and household members, are not eligible to enter or play. Current patients of PHG are also disqualified. "Immediate family members" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses. "Household members" shall mean people who share the same residence at least three months a year. “Current” shall mean any patient(s) who was seen in the 2011 or 2012 calendar year prior to entering sweepstakes.
III. HOW AND WHEN TO ENTER

Entries remain the property of PHGand will not be returned or acknowledged.
Opens to entries at 8:30 AM PST on February 1st, 2012. All entries must be received by 5:00 PM PST on May 30th, 2012.
One entry per person.
Entrants agree to allow Sponsor to contact them with special offers.
IV. HOW AND WHEN WINNERS WILL BE SELECTED AND NOTIFIED
One grand prize winner will be selected at random on or about 5:00 PM PST on May 31st, 2012 from ALL valid entries.
In the event winner is deemed ineligible, fails to comply with sweepstakes procedures, or is not available to claim prize during specified dates, then an alternate winner will be selected at random; the prize will be awarded to runner-up
Winners will be notified by phone from PHG and must redeem prize on April 2nd, 2012 or within one (1) week of being notified before an alternate winner will be selected.
Odds of winning depend on number of valid entries received. Many will enter, only (1) will win “Grand Prize”.
V. PRIZE(S)
One (1) GRAND PRIZE: “Quit for Life” Smoking Cessation Program from PHG ($1400 Value)
Ten (10) RUNNERS UP: Free smoking cessation consultation ($120 Value)
All Entrants will receive a complimentary Self-Mastery Technology session ($50 Value)

VI. RELEASES
By participating, entrants agree to be bound by these official rules and the decisions of the Sponsor/Judges and the sweepstakes administrator, which shall be final in all respects.
By participating in this sweepstakes and accepting any prize that they may win, entrants agree to release Priority Health Group from any and all actions, claims, injury, loss or damage arising in any manner, directly or indirectly, from participation in this sweepstakes and/or acceptance or use of the prize.
Entrants authorize the Released Parties to use their name, voice, likeness (still frame or video), biographical data, city and state of residence and entry materials in programming or promotional material, worldwide in perpetuity, or on a winner’s list, if applicable, without further compensation unless prohibited by law. Sponsor is not obligated to use any of the above mentioned information or materials, but may do so and may edit such information or materials, at Sponsor’s sole discretion, without further obligation or compensation.
VII. MISCELLANEOUS; WINNERS’ LIST
Void where prohibited. All federal, state and local laws and regulations apply.
Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process and to cancel, terminate, modify and/or suspend the contest in the event of tampering or other circumstances, including but not limited to acts of God or civil disturbance that render it impractical or impossible to complete or fulfill the contest as originally planned.
